I need to go to the gynecologist but I've never made an appointment for myself. Do I’ve to go to my primary care doctor first? Does he refer me to someone? Or can I just pick a doctor and call them directly?

If you have an HMO, you probably need to contact your primary care provider (PCP) and get a referral. If you don't, your insurance probably won't pay.
They can either refer you to whoever they normally send people to, or if there’s a Gyno if your network that you want to go to just tell them that. Usually all they’ve to do is give you a paper or sometimes they just call or fax something to the Gyno's office.
Sometimes your PCP can even do the minor gyno stuff, like pap smears as long as everything is normal. Depends on if they’ve a specialty or are just a GP. When I was younger my PCP did them. I let her do it once. She was WAY too happy about getting to do it and it freaked me out a little, so I got a referral the following year to a Gyno i wanted to go to.

It depends on your insurance. If you're on an HMO then you need a referral. If you're on a PPO you can pick any physician you want.
I'd call the gyno office and tell them your insurance and ask if you need a referral otherwise simply schedule the appointment.

I personally would go to my doctor for a refferal, or just look up the golden pages find one and call then and ask if you could perhaps have an app with one of the doc's…

Pick a physician, call them up, make sure they take the type of insurance that you have. And if your just paying for the entire visit in cash. You can go to any one.
